This study aimed at identifying the causes that lead to hospitalization and readmissions in the main hospitals and after that formulate a 30-say implementation practicum that seeks to solve these issues. There has been a growing issue related to hospital hospitalization readmission and hospitalization of patients that have been discharged and permitted to go home and consequently receive treatment there. The problems are caused by releasing patients from the ICU facilities when their time has not yet come. Many health care facilities especially those with ICU facilities usually discharge patients for the wrong reasons that involve a high number of emergency cases that need medical attention. In this case, the medical staff is forced to release and transfer patients to post care programs in such a hurry to take in new emergency cases. This research therefore aimed at coming up with an evidence-based strategy that sought to come up with solutions towards a reduction of readmissions and hospitalization of thirty days discharge. Undeniably, the hospitals achieve this through researching the causes of this admission in their hospitals. The findings were analyzed and evaluated with the aim of coming with an appropriate strategic plan that reduces the hospitalization and readmissions of patients in medical facilities.
On a wider note, the objectives used in carrying out the research include the following:
To identify causes of readmissions and hospitalization of post care patients
To seek solutions that help reduce readmissions and hospitalization
To formulate a comprehensive facility discharge plan seeking to reduce readmissions and hospitalization

Relevant Stakeholders
To ensure that the research outputs from the research informs the research practice and maximizes the benefits to the patients and the nursing hospitals, we intend to work with the patients, physicians, the board of trustee, and insurance company. Fundamentally, these stakeholders will offer appropriate advice and support dissemination to the public. Besides, the information will be gathered and networks created and established throughout this study to further inform and strengthen the dissemination strategy.
Resources Needed
The resource requirements are critical factors for effective dissemination. For this effective oral presentation dissemination plan to be implemented, the presenter needs to have a personal computer and the MS Package, as well as the content that would be included in the presentation. Suitable conference for the project materials, equipment for slide and visual presentation such as projectors will be required. These resources are aimed at maintaining a high quality and professional image for all aspects of the conference including advertising materials, delegates packs, signage, proceedings, etc.

Fundamentally, the process entails the application of PowerPoint slides to an audience. The strategy involves the use of design. The presenter has the option of making one slide for a two-minute presentation. The number of slides made depends on the content being presented. The oral presentation as a proactive dissemination strategy provides a breadth to reach out to multiple audiences and the depth to conduct more in-depth interactive work with the key audiences regarding asking questions, participating in discussions among themselves and the hospital staffs to influence attitudes and behavior change (Kmalvand, 2015).
Apart from being easy to make the PowerPoint designs, this method provides an individual with the ability to communicate effectively to the real world or the target audience, as well as allowing the presenters to use other techniques such as the body language to deliver their message (Knoblauch, 2013). Undeniably, a major component of the finding research message that an audience usually perceives is based on their body language, which integrates the posture, facial expressions, and gestures that affect the manner in which a message is received. It is important to note that other dissemination strategies may also play crucial roles in delivering the research findings as expected. However, through oral presentation dissemination plan, the presenter will be able to use their facial expressions, and vocal tones to speak about something in a bright, confident and assertive manner.
Oral presentation strategy is an effective way of engaging the audience (Knoblauch, 2013). It is an effective strategy of making sure that the message has been received or delivered by the clients. This method of dissemination is entirely different from paper or any other social or print media. The audience, in this case, the case managers, nurses, units directors, quality director, patient liaison, chief nursing officer, chief medical officer, chief executive officer and transitional care coordinator all are given the opportunity to read the message from the researcher himself. The presenter can, therefore, use his knowledge and skills to improve the technique of delivery.
According to Knoblauch (2013), oral presentation dissemination strategy offers an opportunity to deliver the research findings through the provision of interesting examples such as the real-life, facts and figures that support the results of the research. This research has several implications for all the audience. For example, the impact for patients is that they will receive training and access to resources meant to improve patient awareness on their medical conditions. Notably, the patient education plays a significant role in the reduction of readmission rates. Therefore, all patients will receive training and education aimed at improving their self-care and understanding of the care process. Given that the reduction plan seeks to empower patients and increase their awareness, the implications will include increased patient participation to supplement care provision in the hospital. All these can be efficiently delivered through oral presentation strategy.
