They are Apollo Method, the five ways, and Fishbone. The Apollo method is a software that needs a minimum of two causes (Qakley, 2012). On the other hand, the 5 Whys is usually used to obtain a beyond symptoms and in finding the real reason or cause of the accident in question. In this particular scenario, I will use the Fishbone diagrams to determine why the forklift injured the worker. It will be easier to use the Fishbone diagrams for this type of accident since it concentrates on finding the possible causes and grouping them into the required categories. For that case, it will be easier to figure out the reason why the driver of the forklift did not see the worker and if the worker was too close to the forklift (Qakley, 2012).
Question 3: Explanation of The Three Types of Fault Tree Process
Fault tree analysis is used in determining the factors that are connected to fault processes and which lead to a particular event (Qakley, 2012). The first type of fault tree analysis is the onset of the primary function or accident. The second one is to find the event that caused or led to the central event or logic gates, outputs or inputs faults which correspond to the event. The third step is making the process to continue by developing casual factors until the time a correctable event is reached (Qakley, 2012). After finding the root cause of the event, controls are designed to help correct all the casual factors that led to the chain of events which led to the accident.
Question 4: The Problems Associated with effect and cause analysis
Regarding Oakleys (2012) research, the problems that are associated with impact and because the study is that an accident investigation uses several types of effect and case analysis. In most cases, investigators skip some cause of the problem under review and do not look at the issue of training, management, policies or any other factors. Regarding Qakleys (2012) research on safety in engineering, the other problem is that do not follow any analysis through the process hence messing with the causal factors and the corrective actions thus ending up with an uncorrected root cause.
